Arizonans for Children

Mentor Promise

Confidentiality:

Confidentiality is one of the most important things about being a volunteer at Arizonans for Children! Anything that you learn about a client must be kept to yourself. The names, addresses and telephone numbers of foster parents, group homes, children’s school locations or other parties involved must not be given out!

I promise that I shall hold in confidence all case information. I will not violate the confidential relationship between Arizonans for Children, Inc., its volunteers, DCS and its related agencies, the Court and all parties involved. I accept full responsibility for maintaining the confidential and private nature of all records and information. I understand that I am personally responsible and liable for any violation of this agreement. I acknowledge that I have read and signed the full confidentiality agreement.

Abuse Prevention:

All staff, mentors, and other representatives of the program must report any suspected child abuse and/or neglect of agency clients or program participants immediately. A report of suspected child abuse, neglect, exploitation or abandonment is a responsible attempt to protect a child. If a child tells you of such experiences, act to protect that child by calling the toll free Arizona Child Abuse Hotline at 1-888-SOS-CHILD (1-888-767-2445).

I acknowledge that I have read the full abuse prevention policy.

Mentor Commitment:

  • Once matched, I promise to stay with my mentee for a minimum of one year and to meet with my mentee a minimum of 8 hours per month.
  • I promise to submit to my Mentor/Coordinator my monthly log of hours of service, miles driven and activities on a timely basis.
  • I promise to regularly communicate, at least once a month, with my Mentor Coordinator.
